Roles & Responsibilities
- Provide compassionate care coordination and discharge planning for patients
- Conduct thorough psychosocial assessments to support individualized treatment plans
- Collaborate with interdisciplinary teams to ensure seamless patient transitions
- Facilitate 5150 evaluations and involuntary holds when clinically appropriate
- Connect patients and families with vital community resources and support services
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities strongly preferred
- Two supervisory references needed at time of submittal
Knowledge & Skills
- Strong assessment and crisis intervention skills
- Excellent communication and documentation abilities
- Knowledge of involuntary hold processes and mental health regulations
- Ability to work collaboratively within multidisciplinary teams
