Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes across Tehama County
- Conduct Start of Care visits for orthopedic patients when nursing is not ordered
- Develop personalized treatment plans for geriatric patients managing multiple conditions like CHF and COPD
- Complete approximately five meaningful patient visits each day using your clinical expertise
- Document care efficiently with provided tablet and HomeCare HomeBase EMR system
- Collaborate with intake RNs and office managers to coordinate seamless patient care
Qualifications
- Current Physical Therapy license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
- Strong assessment and documentation skills
