Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with every other weekend required

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Coordinate patient care across all hospital departments, ensuring smooth transitions from admission to discharge
Perform comprehensive initial assessments and ongoing care coordination utilizing Interqual and Cerner systems
Collaborate with multidisciplinary teams to develop effective discharge plans for diverse patient populations, including uninsured and homeless patients
Conduct utilization review to ensure appropriate resource utilization and payer compliance
Communicate effectively with patients, families, and caregivers to provide education and support throughout the care journey

Qualifications

Current California RN license in good standing
Minimum of 2 years of recent acute care case management experience, preferably within Dignity Health facilities
BLS certification required
Proficiency with Interqual and Cerner systems is highly preferred; training available for Interqual
Strong clinical assessment and critical thinking skills
Excellent collaboration, teamwork, and communication abilities

Education

Associate's degree in Nursing required; Bachelor of Science in Nursing preferred

Knowledge & Skills

Knowledge of utilization review, discharge planning, and care coordination principles
Familiarity with payer types including Medicare, Medicaid, and managed care
Ability to manage a caseload of 15-20 patients per day
Experience with electronic medical records, especially Cerner
Skill in reprioritizing tasks throughout a dynamic workday
