Location: Woodland, CA

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with rotating Saturday coverage approximately once per month

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in a home health setting
Develop and implement individualized treatment plans
Instruct patients and caregivers in therapeutic exercises and equipment use
Document patient progress and communicate with physicians
Supervise home health aides when involved in patient care
Participate in case conferences and peer consultation

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification required
Excellent oral and written communication skills
Strong organizational skills
Ability to travel within Yolo County service area

Knowledge & Skills

Physical therapy assessment and treatment planning
Home health and community-based care
Patient and caregiver education
Documentation and medical recordkeeping
Interdisciplinary team collaboration
