Contract Type: Travel Contract

Shift Type: Monday - Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide home-based physical therapy services to patients recovering from surgeries, falls, and other conditions
Develop and implement individualized treatment plans to restore function and mobility
Conduct comprehensive patient assessments and document progress in EMR
Collaborate with interdisciplinary team including home health aides and nurses
Educate patients and families on home exercise programs and safety
Travel to patient homes within a 60-mile radius of Valley City

Qualifications

Current BLS certification
North Dakota Physical Therapy License
Valid Driver's License and Auto Insurance
1+ year of experience in physical therapy
Previous home health experience required

Knowledge & Skills

Physical therapy evaluation and treatment techniques
HomeCare/HomeBase EMR proficiency
Home health and hospice patient care
Strong communication and patient education skills
Ability to work independently with no on-site supervision
