Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Perform comprehensive home health evaluations and develop individualized care plans
Conduct start of care, recertifications, and resumption of care visits
Collaborate with interdisciplinary team to ensure optimal patient outcomes
Educate patients and families on home exercise programs and safety
Document patient progress in accordance with OASIS guidelines
Manage a caseload of approximately 5-6 visits per day

Qualifications

Current North Dakota Physical Therapy license
BLS certification (American Heart Association preferred)
Valid driver's license and auto insurance
Previous home health experience and OASIS training preferred
First-time travelers welcome to apply

Knowledge & Skills

Knowledge of home health regulatory requirements and OASIS documentation
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities
Ability to work independently in a home setting
Time management and organizational skills
