Shift Type: Rotating shifts with weekends every 4 weeks and all minor holidays plus 2 major holidays worked
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Provide compassionate end-of-life care to hospice patients in their homes and facilities
- Manage a caseload of 12-14 patients with 4 visits or 2 start-of-care assessments daily
- Collaborate with interdisciplinary teams to ensure comfort and dignity for patients and families
- Document care accurately using Home Care Home Base EMR system
- Support patients and families through the hospice journey with empathy and expertise
Qualifications
- Active California RN license required
- BLS certification required
- Minimum 1 year of hospice experience
- Strong communication and assessment skills
Knowledge & Skills
- Hospice care expertise
- Home care and facility-based patient visits
- Home Care Home Base EMR proficiency
- Compassionate communication with patients and families
- Ability to work independently in the field
