Roles & Responsibilities

Provide skilled nursing care to patients recently discharged from the hospital, managing high-risk conditions to prevent rehospitalization
Conduct start of care, recertification, and resumption of care visits with point-based productivity targets
Educate patients and families on disease management, including CHF, COPD, wounds, wound vacs, and IV/PICC lines
Collaborate with the interdisciplinary team and utilize HomeCare HomeBase (HCHB) for documentation

Qualifications

Valid Registered Nurse (RN) license in California
Current BLS certification
Valid driver's license and auto insurance

Knowledge & Skills

Home health nursing experience preferred
Proficiency in HomeCare HomeBase (HCHB) preferred
Strong patient education and communication skills
