Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate patients' therapeutic needs while developing personalized care plans
Guide patients and families through rehabilitation exercises and therapeutic appliance use
Collaborate with physicians and healthcare teams to optimize patient outcomes
Document progress and communicate changes in patient condition to the care team
Train and supervise home health aides participating in rehabilitation programs
Identify additional home health service needs and coordinate appropriate referrals

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organization abilities
Valid Physical Therapy license for California practice

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Family and caregiver education
Prosthetic device training
Regulatory compliance knowledge including Title 22 and Medicare
Mileage documentation and time management
