Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate each patient's therapeutic needs and develop personalized care plans
Guide patients and families through exercises, mobility training, and use of therapeutic devices
Collaborate with physicians and care teams to track progress and adjust treatment goals
Educate caregivers on how to support rehabilitation between visits
Document clinical notes and progress summaries to keep everyone informed and on track

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from your modality (one within the last 12 months and one within 3 years)

Knowledge & Skills

Home health physical therapy assessment and treatment planning
Patient and family education techniques
Gait, ROM, and mobility evaluation
Prosthetic device training
Compliance with Medicare, Medi-Cal, and Title 22 regulations
Clear documentation and progress reporting
