Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct comprehensive OASIS assessments for accurate care planning
Manage Start of Care cases to establish strong patient relationships
Help elderly patients recover from falls and fractures with personalized treatment plans
Document visits efficiently using tablet technology and point system tracking
Travel between Noble, Lagrange, Steuben, and DeKalb counties to serve diverse communities

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong communication and time management skills

OASIS assessment expertise
Start of Care documentation proficiency
Rural healthcare delivery experience
Tablet-based documentation systems
Geriatric rehabilitation techniques
Fall prevention and fracture recovery protocols
