Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct comprehensive Oasis Assessments and Start of Care evaluations
Help elderly patients recover from falls and fractures with personalized treatment plans
Manage a manageable caseload of 4-6 patient visits daily using modern tablet technology
Build meaningful connections with patients in their home environments

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong communication and time management skills

Knowledge & Skills

Physical Therapy expertise
Oasis Assessment proficiency
Start of Care documentation
Rural healthcare experience preferred
Tablet-based documentation systems
