Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide personalized physical therapy services to patients recovering from surgery, falls, and other conditions in their homes
Develop and implement individualized care plans that help patients regain strength, mobility, and independence
Collaborate with an interdisciplinary team to ensure seamless, patient-centered care
Educate patients and families on safe exercises and techniques to support recovery
Document patient progress accurately using HomeCare/HomeBase EMR
Travel within a 60-mile radius to deliver compassionate care where patients need it most

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of physical therapy experience, including previous home health experience
Valid driver's license and auto insurance
Strong communication and time-management skills

Knowledge & Skills

Home health physical therapy expertise
Comfortable working independently in patients' homes
Familiarity with HomeCare/HomeBase EMR
Ability to manage a caseload of 5-7 patients per day
Knowledge of common home health diagnoses such as post-surgical recovery and fall prevention
