Physical Therapist - Home Health
Location: Santa Maria, California
Start Date: 06-08-2026
End Date: 09-07-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes across assigned geographical regions
- Evaluate and treat patients with orthopedic conditions, strokes, joint replacements, and spinal injuries
- Complete OASIS documentation to meet government-mandated requirements
- Collaborate with interdisciplinary teams including OT and Speech Therapy
- Develop individualized care plans that promote recovery and independence
- Educate patients and families on home exercise programs and safety strategies
Qualifications
- Active Physical Therapist license in California
- BLS certification required
- Minimum 1 year of experience working as a Physical Therapist
- Experience with OASIS documentation required
- Home health setting experience preferred but not required
- Cerner EMR experience preferred
Knowledge & Skills
- Strong assessment and clinical reasoning skills
- Excellent communication and patient education abilities
- Comfortable working independently in home settings
- Proficient with OASIS and Cerner EMR documentation
- Ability to manage a diverse caseload including ortho, neuro, and post-surgical patients
