Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating weekends

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive home health evaluations and develop personalized treatment plans
Provide skilled physical therapy interventions to help patients regain independence at home
Collaborate with an interdisciplinary team for coordinated, patient-centered care
Document patient progress and adjust treatments to maximize outcomes
Educate patients and families on therapeutic exercises and safety strategies

Qualifications

Active Physical Therapist license in Kentucky (or compact eligible)
BLS certification required (American Heart Association)
Home health or acute care experience preferred but not mandatory
Ability to work independently and manage a caseload effectively

Knowledge & Skills

Excellent clinical assessment and problem-solving skills
Strong communication and interpersonal abilities
Knowledge of home health regulations and documentation standards
Time management and prioritization in a dynamic home setting
