Roles & Responsibilities

Screen and evaluate patients with cognitive and ADL challenges to create personalized care plans
Deliver skilled, evidence-based interventions including therapeutic exercises and ADL retraining
Monitor patient progress and adjust treatments to maximize outcomes
Educate patients, families, and caregivers on strategies for daily living and recovery
Collaborate with the care team to coordinate effective treatment plans
Complete accurate documentation and discharge summaries to support continuity of care
Supervise and mentor occupational therapy assistants and rehab aides

Qualifications

Active Occupational Therapist license in Oklahoma
At least 1 year of OT experience in long-term care or rehab settings
New grads welcome with strong Level II fieldwork experience
Strong clinical skills in patient evaluation, treatment planning, and documentation

Knowledge & Skills

Occupational Therapy expertise
Patient evaluation and treatment planning
ADL retraining and therapeutic exercises
Supervising OT assistants and coordinating DME
Electronic documentation and regulatory compliance
