Shift Type: Monday-Friday, 8:30 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless care transitions for patients and families
Collaborate with interdisciplinary teams to develop personalized discharge plans
Utilize Midas and Meditech systems to track and manage patient cases
Advocate for patients while navigating complex healthcare systems
Ensure safe and timely discharges with appropriate resources in place

Qualifications

Active RN license in Florida or compact license
Minimum 1 year of case management experience
Hands-on experience with Midas and Meditech systems
Strong background in discharge planning preferred

Knowledge & Skills

Proficient with Midas and Meditech electronic systems
Skilled in complex discharge planning strategies
Excellent communication and collaboration abilities
Detail-oriented with strong organizational skills
