Roles & Responsibilities

• Provide direct patient care and support rehabilitation goals for patients in a skilled rehab facility
• Administer medications and treatments as ordered, monitoring patient responses
• Collaborate with the interdisciplinary care team to develop and implement individualized care plans
• Document patient assessments, interventions, and outcomes accurately and timely
• Educate patients and families on treatment plans, medications, and discharge instructions
• Ensure a safe, compassionate environment that promotes patient dignity and independence

Qualifications

• Valid LPN or RN license in Maryland
• Current BLS/CPR certification
• COVID-19 vaccination or documentation of religious/medical exemption per facility requirements
• Ability to provide a state or federal issued ID and wear a photo badge at all times
• Ability to work the scheduled shift from 7:00 AM to 3:30 PM

• Clinical nursing skills including medication administration and patient assessment
