Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration, wound care, and monitoring vital signs
• Collaborate with the healthcare team to develop and implement individualized care plans
• Document patient care accurately and in a timely manner
• Assist residents with activities of daily living, promoting independence and dignity
• Educate patients and families on health conditions and care plans
• Respond to patient needs and changes in condition, reporting to the RN or provider

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Vermont
• Current BLS certification (American Heart Association)
• Experience in long-term care, skilled nursing, or rehabilitative settings
• Must have reliable transportation to and from the facility
• Must have completed COVID-19 vaccination series or have a documented religious/medical exemption
• Ability to commit to the full 5-week contract period

• Proficient in medication administration and dosage calculations
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Knowledge of electronic health records (EHR) documentation
• Ability to work effectively in a team environment
• Compassionate and patient-centered approach to care
