Roles & Responsibilities

• Provide compassionate, direct patient care in an acute rehabilitation setting
• Administer medications and treatments as ordered
• Monitor and document patient vital signs and conditions
• Collaborate with the care team to develop and adjust care plans
• Educate patients and families on health management and recovery
• Respond promptly to changes in patient status and escalate concerns

Qualifications

• Current, valid LPN license in the state of Texas
• Current BLS certification (American Heart Association preferred)
• Previous acute care or rehabilitation experience is a plus
• Ability to assess patient needs and prioritize care
• Strong clinical and documentation skills
• Compassionate, reliable, and team-oriented

Knowledge & Skills

• Knowledge of LPN scope of practice and nursing fundamentals
• Medication administration and IV therapy skills
• Understanding of acute rehabilitation protocols
• Proficiency with electronic health records (EHR)
• Effective communication with patients, families, and team members
• Ability to adapt in a fast-paced environment
