Roles & Responsibilities

• Conduct comprehensive admission assessments and Start of Care visits to establish individualized care plans
• Perform routine visits, recertifications, and resumptions of care while tracking patient progress
• Administer and manage medications, including IV infusions and injections, with safe medication reconciliation
• Provide skilled wound care and Wound V.A.C. management to support healing and prevent complications
• Manage ostomy and G-tube care and educate patients and caregivers for safe home management
• Use OASIS, EPIC Remote Client (laptop) and Rover (phone app) for timely, compliant documentation
• Collaborate with the interdisciplinary team and utilize interpretation services to ensure equitable care
• Apply infection control precautions and patient education to reduce infection risk and improve outcomes

Qualifications

• Current RN license (Oregon licensure preferred; pending license accepted)
• Minimum 1 year of skilled home health nursing experience (first-timers considered)
• BCLS/BLS certification required
• Demonstrated experience with OASIS preferred — please note if you do not have OASIS experience
• Comfort with EPIC Remote Client and mobile documentation (Rover app) strongly preferred
• Own transportation and willingness to drive within a typical daily radius (occasional max ~60 miles)
• Facility requires primary COVID vaccine series plus one booster

Education

• Active Registered Nurse (RN) license (Oregon or eligible for Oregon; pending accepted)
• BCLS/BLS certification (required)

Knowledge & Skills

• Home health clinical practices and patient-centered assessments
• OASIS data collection and documentation
• EPIC Remote Client charting and Rover mobile documentation
• Medication administration and IV/IM/SubQ injection techniques, including IV infusions
• Wound care, Wound V.A.C. management and integumentary assessment
• Colostomy/ileostomy and G-tube care
• PICC line familiarity and catheter care/education
• Pain assessment/management and PCA pump experience
• Infection control precautions and safe home visit practices
• Disease management and patient/caregiver education
