Roles & Responsibilities

Provide compassionate nursing care to patients in their homes or hospice settings
Assess patient conditions and implement care plans under the direction of a registered nurse
Administer medications and treatments as prescribed
Educate patients and families on health management and end-of-life care
Document patient progress and communicate with the interdisciplinary team
Maintain a safe and comfortable environment for patients

Qualifications

Valid LPN license in the state of Michigan
Current BLS certification
Prior home care or hospice experience is a plus, but we welcome new graduates
Ability to work independently and as part of a team
Strong interpersonal and communication skills

Knowledge & Skills

Skilled in patient assessment and clinical judgment
Knowledge of medication administration and wound care
Comfortable with electronic health records
Ability to adapt to different patient environments
Empathy and patience when working with patients and families
