Roles & Responsibilities

• Conduct thorough screening, assessment, diagnosis, treatment planning, brief therapy, and continuing care for patients from birth to elders
• Lead discharge planning and facilitate patient care conferences for seamless transitions to ALF or other placements
• Provide expert care coordination, client advocacy, and skill-building to strengthen families and support cultural wellness
• Collaborate with multidisciplinary hospital teams and community partners for optimal patient outcomes
• Maintain precise, compliant documentation including progress notes, treatment plans, and reports to meet all standards
• Deliver crisis intervention, case management, and referrals for chemical dependency and mental health needs

Qualifications

• Masters Degree in Social Work, Psychology, Counseling or related discipline from an accredited institution
• Licensed as or eligible for Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT)
• Valid State of Alaska Driver’s License or ability to obtain within 6 months of hire
• At least 21 years of age
• Clinical Competency required within 3 months of hire and every three years

Education

• Masters Degree in Social Work, Psychology, Counseling or related discipline from an accredited educational institution

Knowledge & Skills

• Expertise in discharge planning, interviewing techniques, and crisis intervention
• Strong case management with tact, discretion, and independent judgment
• Skilled in observing and recording behaviors, conducting assessments, and developing treatment plans
• Proficient in analyzing situations, ethical practices, and legal patient rights
• Ability to write clear, concise reports and documentation per professional and regulatory standards
