Roles & Responsibilities

Coordinate seamless patient care transitions to optimize outcomes
Master complex discharge planning for safe patient returns home
Collaborate with multidisciplinary teams to navigate care pathways
Utilize Meditech and Midas systems to track and document progress
Advocate fiercely for patients ensuring their needs are met

Qualifications

Active RN license in good standing
Minimum 1 year of case management experience
Proficiency in Midas and Meditech systems required
Florida RN license or compact state license preferred

Education

RN degree from accredited nursing program

Knowledge & Skills

Expertise in RN case management
Strong skills in complex discharge planning
Comfortable with high patient ratios and dynamic environments
Team player who thrives in collaborative settings
