End Date: 09-22-2026

Contract Type: Contract

Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, up to every other weekend

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Manage a meaningful caseload of 5-6 new palliative patients and 3-4 follow-ups daily
Lead compassionate goals-of-care discussions and family meetings
Educate patients and families on disease processes, advance directives, and POLSTs
Collaborate during daily rounds with physicians and the interdisciplinary team
Review patient charts and assess needs to deliver holistic, patient-centered care
Provide emotional support and guidance during end-of-life conversations

Qualifications

Active California RN license
BLS certification required
ELNEC certification preferred
2-5 years recent palliative or hospice experience
Acute care background strongly preferred
ICU or oncology experience a plus

Knowledge & Skills

Strong clinical thinking and communication skills
Comfortable using Cerner EMR
Excellent organizational skills and flexibility
Passion for serving patients, families, and team members
Ability to navigate multiple hospital units with ease
