Shift Type: Rotating shifts with weekends required approximately every 4 weeks and all minor holidays plus 2 major holidays worked
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Provide compassionate end-of-life care to hospice patients in their homes and facilities
- Manage a caseload of 12-14 patients with 4 visits or 2 start-of-care assessments daily
- Build trusting relationships with patients and families during their most important moments
- Document care accurately using Home Care Home Base EMR system
- Collaborate with interdisciplinary teams to ensure comfort and dignity for every patient
Qualifications
- Active California RN license required
- Minimum 1 year of hospice experience
- Current BLS certification
- Strong communication and assessment skills
- Ability to work independently in home and facility settings
Knowledge & Skills
- Hospice care principles and end-of-life support
- Home care assessment and documentation
- Patient and family education
- Comfort care and symptom management
- Electronic medical records (Home Care Home Base)
