Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every other weekend rotation
Hours per Week: 40
Candidate Pay Rate: $82 per hour

Roles & Responsibilities
- Coordinate comprehensive care plans for 15-20 patients daily across all hospital areas
- Conduct initial assessments and utilization reviews using Interqual criteria
- Collaborate with interdisciplinary teams to ensure seamless patient transitions
- Develop discharge plans that address the unique needs of diverse patient populations
- Navigate complex payer requirements while advocating for optimal patient outcomes
- Build meaningful connections with patients facing challenging circumstances
Qualifications
- Active California RN license required
- BLS certification required
- Prior recent experience with Dignity Health facilities strongly preferred
- Experience with Interqual criteria and Cerner EMR preferred
- Strong collaboration and communication skills essential
- Ability to adapt and reprioritize throughout dynamic workdays
Knowledge & Skills
- Case management and care coordination expertise
- Utilization review and discharge planning proficiency
- Interqual criteria knowledge (training provided)
- Cerner electronic medical record system experience
- Excellent teamwork and communication abilities
- Experience working with diverse patient populations and payers
