Physical Therapist - Home Health
Location: Santa Maria, California
Start Date: 06-07-2026
End Date: 09-06-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes across assigned geographic regions
- Evaluate and treat patients with orthopedic conditions, strokes, joint replacements, and spinal injuries
- Complete OASIS documentation accurately and timely as required by government standards
- Collaborate with interdisciplinary teams including OT and Speech therapists for comprehensive care
- Provide patient education and support to promote recovery and independence
- Maintain professional appearance with business attire and white lab coat
Qualifications
- Active Physical Therapist license in California
- BLS certification required
- Minimum one year of experience as a Physical Therapist
- Experience with OASIS documentation required
- Home health experience preferred but not required
- Comfortable working independently in home-based care settings
Knowledge & Skills
- Strong assessment and clinical reasoning skills
- Proficiency with Cerner EMR system
- Excellent communication and patient education abilities
- Ability to manage a diverse caseload including ortho, neuro, and post-surgical patients
- Flexible and adaptable approach to scheduling and patient needs
