Location: Red Bluff, California

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes across Tehama County
- Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
- Develop personalized treatment plans for patients recovering from joint replacements and managing chronic conditions
- Help geriatric patients with multiple comorbidities like CHF and COPD improve their mobility and quality of life
- Document patient progress using HomeCare HomeBase EMR system on provided tablet
- Collaborate with intake RN and office manager to coordinate care effectively
Qualifications
- Current Physical Therapist license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience
- Experience with HomeCare HomeBase EMR system preferred
- Valid driver's license and reliable transportation
Knowledge & Skills
- Home Health PT expertise
- Proficiency with HomeCare HomeBase EMR
- Strong assessment and clinical reasoning skills
- Excellent communication and patient education abilities
- Comfortable working independently in home settings
