Roles & Responsibilities

• Administer medications and treatments as prescribed by physicians
• Monitor patient vitals and report changes to the nursing team
• Assist patients with daily living activities such as bathing, dressing, and mobility
• Maintain accurate patient documentation and care plans
• Collaborate with nurses, therapists, and doctors to ensure quality care
• Provide compassionate support to patients and their families

Qualifications

• Active LPN license in New Hampshire (or compact state eligibility)
• Current BLS/CPR certification
• Previous experience in rehab, long-term care, or skilled nursing preferred
• COVID-19 vaccination or approved exemption required
• Ability to pass a background check and provide government-issued ID

Knowledge & Skills

• Strong clinical assessment and observation skills
• Proficiency in medication administration and dosage calculations
• Excellent communication and interpersonal skills
• Ability to work effectively in a team-oriented environment
• Knowledge of electronic health records (EHR) systems
• Compassionate and patient-centered approach to care
