Roles & Responsibilities

• Provide compassionate, patient-centered care to residents and patients
• Administer medications and treatments as prescribed by physicians
• Monitor and document vital signs, symptoms, and changes in patient condition
• Collaborate with RNs and the interdisciplinary healthcare team to ensure quality care
• Educate patients and families on treatment plans and health management
• Maintain accurate and timely patient records and documentation

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of Vermont
• CPR/BLS certification required
• Must have own transportation - area has limited public transportation
• COVID-19 vaccination or approved medical/religious exemption required
• Willingness to work every weekend
• Prior LPN experience in long-term care or rehabilitation preferred
