Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and treatments
• Monitor and document patients' vital signs and health status
• Collaborate with RNs and physicians to implement individualized care plans
• Assist residents with daily living activities and ensure their comfort
• Educate patients and families on health management and care plans
• Maintain accurate, timely documentation in compliance with facility policies
• Respond promptly to patient needs and escalate concerns as appropriate

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification (American Heart Association preferred)
• Strong clinical assessment and critical thinking skills
• Prior experience in long-term care, skilled nursing, or geriatrics is a plus
• Excellent communication and interpersonal skills
• Ability to work evening shifts as scheduled

Knowledge & Skills

• Knowledge of nursing principles, pharmacology, and patient care best practices
• Skilled in using electronic health records (EHR) systems
• Strong attention to detail and ability to multitask
• Compassionate bedside manner with a patient-centered approach
• Effective communication with patients, families, and the care team
• Ability to work independently and as part of a multidisciplinary team
