Roles & Responsibilities
- Provide compassionate nursing care to residents in a skilled nursing facility
- Administer medications and treatments as prescribed
- Monitor patient vital signs and report changes to the healthcare team
- Assist with activities of daily living and ensure patient comfort
- Collaborate with RNs, physicians, and other staff to deliver high-quality care
- Maintain accurate patient records and documentation
Qualifications
- Current LPN license in the state of Connecticut
- Experience in long-term care or skilled nursing preferred
- BLS certification required
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
Knowledge & Skills
- Proficient in medication administration and wound care
- Knowledge of electronic health records (EHR) systems
- Excellent time management and organizational skills
- Compassionate and patient-centered approach
