Roles & Responsibilities

• Provide compassionate, patient-centered care to residents in a skilled nursing facility
• Administer medications and treatments as ordered, observing and reporting any changes in patient condition
• Monitor vital signs and document patient progress accurately
• Assist with daily living activities and support patients' comfort and dignity
• Collaborate with registered nurses, physicians, and the care team to deliver high-quality outcomes
• Maintain a safe, clean, and welcoming environment for patients and families

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of Pennsylvania
• Current BLS/CPR certification
• Prior LPN experience in long-term care, skilled nursing, or a similar setting is preferred
• Ability to work flexible per diem shifts, including evenings and nights
• Strong critical thinking and communication skills with a team-oriented mindset

Knowledge & Skills

• Knowledge of nursing principles, medication administration, and patient assessment
• Proficiency in electronic health records and documentation
• Strong attention to detail and ability to multi-task in a fast-paced setting
• Excellent interpersonal skills with patients, families, and team members
• Understanding of infection control and safety protocols
