Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents and patients
• Administer medications and treatments accurately and safely following physician orders
• Monitor vital signs and overall condition, promptly reporting any changes to the healthcare team
• Document patient care thoroughly and maintain accurate medical records
• Collaborate with nurses, doctors, and other team members to ensure the best possible outcomes
• Educate patients and families on care plans and wellness practices

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification (or ability to obtain before starting)
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Compassionate and dedicated to delivering high-quality care
• Previous long-term care or skilled nursing experience is a plus, but not required

Knowledge & Skills

• Solid knowledge of nursing principles and best practices
• Proficient in medication administration and dosage calculations
• Comfortable with electronic health records (EHR) documentation
• Skilled in patient assessment and monitoring
• Able to communicate warmly and effectively with patients and families
• Flexible and adaptable in a team environment
