Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at Andover Manor Rehab & Nursing
• Administer medications and treatments accurately and timely
• Monitor and document vital signs, changes in condition, and response to treatment
• Collaborate with nurses, therapists, and care staff to create and follow individualized care plans
• Assist with activities of daily living while promoting resident dignity and independence
• Maintain clear, timely clinical documentation in the facility charting system
• Escalate clinical concerns promptly to ensure patient safety and quality outcomes

Qualifications

• Valid Licensed Practical Nurse (LPN) license in Massachusetts required
• Current BLS/CPR certification preferred or required per facility policy
• Previous experience in long-term care, skilled nursing, or rehab settings preferred
• Strong communication and teamwork skills; ability to advocate for residents
• Ability to pass background checks and meet facility onboarding requirements

Education

• Completed LPN diploma or vocational nursing program

Knowledge & Skills

• Skilled in medication administration and basic wound care
• Comfortable performing resident assessments and charting clinical observations
• Knowledge of infection control practices and patient safety protocols
• Able to work within an interdisciplinary team and follow care plans
• Familiarity with electronic medical records (EMR) and documentation best practices
