Roles & Responsibilities

• Provide direct patient care including assessments, medication administration, and treatments with compassion and precision
• Monitor patients' conditions and document vital signs, symptoms, and progress accurately in PCC
• Collaborate with nurses, physicians, and care team members to develop and adjust care plans
• Educate patients and families on care plans, medications, and post-treatment instructions
• Ensure a safe, comfortable environment by following infection control and safety protocols

Qualifications

• Active LPN license in Vermont or hold a compact nursing license (NLC) if applicable
• Must have hands-on experience with PCC (PointClickCare) - this is essential
• Current BLS/CPR certification
• Availability to work every other weekend and some holidays
• Strong clinical assessment and critical thinking skills

Knowledge & Skills

• Solid understanding of LPN scope of practice and long-term care nursing
• Proficiency with PCC (PointClickCare) electronic health records
• Excellent communication and interpersonal skills for teamwork and patient interaction
• Ability to maintain composure and make sound decisions in fast-paced situations
• Compassionate, patient-centered approach to care
