Roles & Responsibilities

• Conduct comprehensive home health assessments for patients with orthopedic, stroke, joint, and spinal conditions
• Develop and implement individualized occupational therapy care plans focused on improving daily living skills
• Deliver skilled therapeutic interventions in the home setting to promote independence and safety
• Collaborate with the interdisciplinary care team to coordinate patient-centered care
• Complete OASIS documentation accurately and timely using Cerner EMR
• Educate patients and caregivers on adaptive techniques and home exercise programs

Qualifications

• Active California Occupational Therapist license (or ability to obtain prior to start)
• BLS Certification required
• Minimum 2 years of experience as an Occupational Therapist
• Home health experience preferred, but not required
• Must be comfortable with OASIS documentation
• Valid driver's license and reliable transportation for home visits

Knowledge & Skills

• Proficiency with Cerner EMR
• Understanding of OASIS assessment and documentation requirements
• Knowledge of ortho, stroke, joint, and spinal injury rehabilitation
• Strong clinical reasoning and communication skills
• Ability to work independently and manage a geographically assigned caseload
