Roles & Responsibilities

• Provide compassionate nursing care to residents in a long-term care setting
• Administer medications, treatments, and injections accurately and safely
• Monitor patient conditions and report any changes to the healthcare team
• Collaborate with RNs, physicians, and other staff to develop and implement care plans
• Maintain detailed and accurate patient records and documentation
• Educate residents and their families on health conditions and care procedures

Qualifications

• Active LPN license in Vermont or compact state license
• BLS certification from AHA or equivalent
• Previous LPN experience in long-term care or skilled nursing is preferred
• Ability to work night shifts, weekends, and holidays
• Strong clinical assessment and documentation skills

• Medication administration and dosage calculation

• Electronic health records (EHR) documentation
