Shift Type: Monday-Friday, 8:00 AM - 4:30 PM with rotating weekends
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Coordinate seamless discharge planning for patients transitioning from acute care
- Collaborate with physicians, social workers, and care teams to ensure optimal patient outcomes
- Advocate for patients and families throughout their healthcare journey
- Utilize Meditech, Midas, and Wellsky systems to document and track care plans
- Float to various units as needed to support hospital-wide case management needs
Qualifications
- Active RN license in Missouri or compact state
- Minimum 1 year of recent acute care discharge planning experience
- Strong communication and organizational skills
- Ability to work collaboratively within a multidisciplinary team
Knowledge & Skills
- Discharge planning expertise in acute care settings
- Familiarity with Meditech, Midas, and Wellsky documentation systems
- Understanding of patient ratios and care coordination
- Business casual professional presentation
