Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's functional status to create tailored care plans
Guide patients and families on therapeutic exercises and equipment use
Monitor progress and communicate updates to physicians and care teams
Coordinate with home health aides to support rehabilitation goals
Document clinical findings and celebrate patient milestones

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Family and caregiver education
Medicare and Medi-Cal compliance knowledge
Clear documentation and reporting abilities
