Roles & Responsibilities

• Evaluate and assess patients physical abilities and limitations
• Develop individualized treatment plans to improve mobility and function
• Provide hands-on therapy and therapeutic exercise
• Educate patients and families on home exercise programs and safety
• Collaborate with an interdisciplinary team to coordinate care
• Document patient progress accurately and timely
• Help patients regain independence and improve quality of life
• Supervise physical therapy assistants and aides as needed

Qualifications

• Active California Physical Therapist license or eligibility to obtain
• Degree in Physical Therapy from an accredited program
• BLS certification or willingness to obtain
• New grads welcome; post-acute experience is a plus
• Strong communication and patient care skills
• Ability to work collaboratively in a team environment

Knowledge & Skills

• Knowledge of physical therapy principles and evidence-based practices
• Skilled in patient evaluation and treatment planning
• Familiarity with post-acute or skilled nursing settings
• Excellent interpersonal and communication abilities
• Strong organizational and time management skills
• Compassion and a patient-centered mindset
