Roles & Responsibilities

• Provide compassionate direct patient care and monitor vital signs, reporting any changes to the care team
• Administer medications and treatments as ordered by physicians while ensuring patient safety and comfort
• Conduct patient assessments and document accurate medical information in a timely manner
• Collaborate with nurses, doctors, and other healthcare professionals to deliver high-quality outcomes
• Educate patients and families on care plans and post-discharge instructions

Qualifications

• Current and active LPN or RN license in Pennsylvania
• Current BLS/CPR certification
• Previous clinical experience in a hospital or long-term care setting preferred
• Excellent critical thinking and communication skills
• Must be able to pass a background check and drug screening

• Strong clinical assessment and patient care skills
• Proficient in medication administration and documentation
• Ability to prioritize tasks in a fast-paced environment
