Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Perform thorough physical therapy evaluations and establish individualized plans of care
- Provide skilled therapy across settings — patient room, hallway, outdoor areas and the rehab gym
- Collaborate closely with the rehab team, nursing and care managers during rounds and care conferences
- Document interventions, progress and discharge planning accurately in Cerner and NetHealth
- Assist with discharge planning and patient/family education to support safe transitions
- Adapt care for acute, swing bed and nursing home patients while maintaining patient safety and ratios
- Contribute to unit initiatives, support the growth of the rehab gym, and share clinical expertise
Qualifications
- Current NC Physical Therapy license (required)
- CPR certification (required)
- Minimum 2 years of clinical experience (position lists 2 years); clinicians with 3+ years in a medical setting are preferred
- Experience in hospital and/or nursing home settings (must have)
- Comfort floating between acute/swing, nursing home and outpatient settings as needed
- Experience documenting in Cerner and NetHealth (documentation experience strongly preferred)
Knowledge & Skills
- Skilled in physical therapy evaluations, plan of care development, and discharge planning
- Strong documentation practices using Cerner and NetHealth
- Experience with typical rehab equipment and therapy in diverse spaces (gym, rooms, outdoors)
- Excellent communication and collaboration with interdisciplinary teams
- Ability to manage 1:1 patient care and prioritize caseload in a rehab-focused hospital
