Roles & Responsibilities

Assist licensed therapists in delivering patient-centered treatment plans in a Skilled Nursing Facility
Implement therapeutic exercises and modalities to help patients regain mobility and function
Document patient progress accurately and communicate changes to the care team
Support safe patient transfers and mobility, prioritizing dignity and independence
Collaborate with nursing and interdisciplinary staff to coordinate excellent care
Educate patients and families on home exercise programs and safe mobility strategies
Contribute to a positive rehab environment that motivates patients toward goals

Qualifications

Experience or training as a PTA (Physical Therapy Assistant) — new grads encouraged and welcome
Comfort working in a Skilled Nursing Facility (SNF) practice setting
Ability to work daytime shifts and collaborate with an interdisciplinary team
Strong communication and documentation skills; professional, compassionate bedside manner
Credentials/licensure as required by state regulations (if applicable)

Knowledge & Skills

Understanding of rehabilitation principles and therapeutic exercise
Familiarity with SNF workflows and patient safety protocols
Skill in patient transfers, gait training, and use of assistive devices
Good clinical judgment and ability to follow care plans closely
Strong teamwork and patient education skills
