Roles & Responsibilities

• Provide compassionate and skilled physical therapy services to patients in a skilled nursing setting
• Develop individualized treatment plans to help patients improve mobility, strength, and independence
• Collaborate with nurses, physicians, and therapists to ensure coordinated and effective patient care
• Monitor patient progress and adjust treatment interventions as needed
• Educate patients and their families on therapeutic exercises and home safety strategies
• Maintain accurate documentation in accordance with facility and regulatory standards

Qualifications

• Active Physical Therapist license in New Hampshire or compact eligibility
• Graduate of an accredited physical therapy program (DPT or Master's)
• Previous experience in skilled nursing, rehabilitation, or long-term care preferred
• Current BLS certification
• Strong clinical reasoning, assessment, and documentation skills
• Ability to build trusting relationships with patients and families

Knowledge & Skills

• Knowledge of evidence-based physical therapy practices and modalities
• Proficiency with electronic medical records (EMR) systems
• Excellent communication and interpersonal skills
• Physical ability to assist patients with transfers and mobility tasks
• Strong time management and organizational abilities
• Compassionate, patient-centered approach to care
