Candidate Pay Rate: $69 per hour

Roles & Responsibilities
- Provide compassionate and skilled physical therapy services to patients in a skilled nursing setting
- Develop individualized treatment plans to help patients improve mobility, strength, and independence
- Collaborate with nurses, physicians, and therapists to ensure coordinated and effective patient care
- Monitor patient progress and adjust treatment interventions as needed
- Educate patients and their families on therapeutic exercises and home safety strategies
- Maintain accurate documentation in accordance with facility and regulatory standards
Qualifications
- Active Physical Therapist license in New Hampshire or compact eligibility
- Graduate of an accredited physical therapy program (DPT or Master's)
- Previous experience in skilled nursing, rehabilitation, or long-term care preferred
- Current BLS certification
- Strong clinical reasoning, assessment, and documentation skills
- Ability to build trusting relationships with patients and families
Knowledge & Skills
- Knowledge of evidence-based physical therapy practices and modalities
- Proficiency with electronic medical records (EMR) systems
- Excellent communication and interpersonal skills
- Physical ability to assist patients with transfers and mobility tasks
- Strong time management and organizational abilities
- Compassionate, patient-centered approach to care
