Setting: Skilled Nursing Facility (SNF)
Contract: 13 Weeks
Pay: $65–$68/hour
Start Date: ASAP

We’re looking for a Physical Therapist (PT) to join a Skilled Nursing Facility in Yountville, CA for a 13-week contract assignment.

What You’ll Do

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises, gait training, balance, and mobility interventions
• Monitor patient progress and modify treatment plans as needed
• Help residents improve strength, mobility, function, and independence
• Maintain accurate and timely clinical documentation
• Collaborate with PTAs, nursing staff, and the interdisciplinary team
• Provide compassionate, patient-centered care

What We’re Looking For

• Active California PT license
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• SNF experience preferred
• Strong communication and clinical documentation skills
