Roles & Responsibilities

• Perform thorough physical therapy evaluations and develop individualized treatment plans
• Deliver skilled interventions to improve mobility, strength, pain management, and function
• Document patient progress clearly and timely in the medical record
• Collaborate with nurses, case managers, and families to achieve safe discharges and meaningful outcomes
• Educate patients and caregivers on home exercise programs and injury prevention
• Participate in quality improvement efforts and maintain compliance with facility policies

Qualifications

• Current Physical Therapist license in Massachusetts or ability to obtain licensure promptly
• Graduation from an accredited Physical Therapy program; DPT preferred
• Strong clinical assessment skills and experience with post-acute or rehab populations
• Excellent communication and teamwork skills with a patient-centered approach
• Current BLS/CPR certification or willingness to obtain prior to start
• Ability to work the scheduled days, including weekends as required by the assignment

Education

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Knowledge & Skills

• Skilled in patient evaluation, therapeutic exercise, manual therapy, and gait training
• Comfortable with documentation systems and regulatory compliance
• Knowledge of adaptive equipment, transfer techniques, and safe mobility training
• Strong clinical reasoning and the ability to modify plans based on patient response
• Ability to work efficiently in a fast-paced post-acute/rehab setting
• Good time management and patient education skills
