Shift Type: Monday-Friday, 8:30 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $58 per hour

Roles & Responsibilities
- Provide personalized physical therapy services in patients' homes across Yolo County
- Assess and evaluate each patient's therapeutic needs and develop individualized care plans
- Guide patients and families through exercises, mobility training, and use of therapeutic equipment
- Collaborate with physicians and care teams to track progress and adjust treatment goals
- Support patients in achieving greater independence and improved quality of life
Qualifications
- Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
- Current BLS certification required
- Strong communication and organizational skills
- Valid driver's license and reliable transportation for home visits
Knowledge & Skills
- Home health physical therapy expertise
- Patient assessment and care planning
- Family and caregiver education
- Documentation and clinical reporting
- Knowledge of Medicare and Medi-Cal regulations
