Candidate Pay Rate: $59 per hour

Roles & Responsibilities

  • Deliver personalized physical therapy services in patients' homes
  • Create customized treatment plans that help patients regain mobility and independence
  • Collaborate with interdisciplinary care teams to ensure seamless patient outcomes
  • Educate patients and families on exercises and recovery techniques
  • Document progress and adjust care plans to meet individual goals

Qualifications

  • Active California Physical Therapist license
  • BLS certification
  • Home health experience preferred
  • Strong communication and time-management skills

Knowledge & Skills

  • Patient assessment and treatment planning
  • Home safety evaluation
  • Therapeutic exercise instruction
  • Progress documentation
  • Care coordination with healthcare teams
